Proper Identification and documentation is required for services to be provided.  Categories below list the required documents for identification card issuance and DEERS enrollment:

 The Common Access Card (CAC), also known as the Smart Card, is the primary focus of the SET-D program. CAC is an identification card with a tiny personal database embedded in it that can be used for access to buildings, networks, and email.  CAC's are issued to all Active Duty/Reserve/Guard/DOD Civilians/DOD Contractors. Department of Homeland Security (DHS) has established new rules for documents that will be required for Identification and Issuance/Re-issuance of CAC Cards. See table below:

 

TO RECEIVE A DEPARTMENT OF DEFENSE IDENITIFICATION CARD OR A CAC CARD ALL PERSONNEL MUST BRING TWO FORMS OF APPROVED ID, ONE OF WHICH MUST BEAR A PHOTO.  A CURRENT OR EXPIRED ID CARD/CAC IS CONSIDERED A VALID FORM OF ID.  LIST A AND B ARE APPROVED IDENTIFICATION DOCUMENTS

Expired Cards:  Individual will need the expired CAC card in addition to a second document from the table above. You may obtain a new CAC Card up to 60 days prior to expiration.   

Grade Change:  Promotion Order, current CAC Card and 1 additional document from the list above

MP Confiscation:  Carlisle Barracks Form 120 (ID Card receipt) issued by the Carlisle Barracks Civilian Police and individuals will be required to have 2 forms of identification from the table above - one must have a photo.

Lost ID Card:  Police report from the Carlisle Barracks Police Office (bldg# 400 ) and individuals will be required to have 2 forms of identification from the table above - one must have a photo.

Reserve/National Guard:  Initial issue of ID card requires a copy of the enlistment contract, for renewal a copy of the latest enlistment contract.  A DD Form 214 can be used if it reflects the reserve obligation date and individuals will be required to have 2 forms of identification from the table above - one must have a photo.

DOD Civilian Employees:  Requires two forms of identification from the list above, the expired CAC can be one of the documents. Department of the Army civilian employees must have an AKO account or other working email address.

Individual Ready Reserve:  An enlistment contract or commissioning oath of office and individuals will be required to have 2 forms of identification from the table above - one must have a photo.

Contractors:  Must have been entered into DEERS through the Contractor Verification System (CVS).  Must have 2 forms of identification from the list above, an expired CAC card can be used as one of the documents.  

All other Identification Cards:

Retirees:  Retirement Order or DD Form 214 current CAC/ID Card and one of the documents listed in the above table.   

Spouse: Original or certified copy of marriage certificate, birth certificate, social security card and individuals must present one additional document with photo.   

Children Born out of Wedlock (Army only): Sponsor must have a court order that establishes paternity and the child's birth certificate or approved dependency and residency determination from DFAS (requesting forms may be obtained from the ID Card office) with birth certificate or a notarized voluntary acknowledgement of paternity from the state the child resides in and birth certificate, the sponsor must be present to sign the DD Form 1172 (ID Card application) or the spouse must have the sponsor's Power of Attorney to sign for the sponsor in his/her absence or the spouse must have a DD Form 1172 not more than 90 days old signed by the sponsor and verified by his/her servicing ID Card Office.  A copy of the deployment orders if the sponsor is deployed.  All others must process their request through their respective branch of service. 

Child, Age 21-23:   Child must be enrolled in college as a full-time student.  A letter from the school certifying that the student is enrolled full-time and must provide the start date and expected graduation date.  Letter must be on school letterhead.  The sponsor must be present to the sign the DD Form 1172 or the student/spouse must have the power of attorney to sign for the sponsor; a valid DD Form 1172 signed by the sponsors servicing ID card facility not more than 90 days old; a copy of the sponsors deployment order.  Navy and USMC personnel must go thru their branch of service.

Newborns:   All newborns must be added to DEERS soon as possible.  A hospital record of birth (signed by the attending physician) or a birth certificate is required.  A social security card must be presented as soon as one is issued.  The sponsor must be present to sign the DD Form 1172  or the spouse must have the sponsor's Power of Attorney to sign for the sponsor in his/her absence or the spouse must have a DD Form 1172 not more than 90 days old signed by the sponsor and verified by his/her servicing ID Card Office.  A copy of the deployment orders if the sponsor is deployed.

DOD Beneficiaries (Army only) :  Unremarried former spouses can continue to keep their dependent ID card privileges provided they meet the following criteria:  The member must have served at least 20 years creditable military service for retired pay, the marriage lasted at least 20 years, and the marriage and service time overlapped by at least 15 years.  The number of years overlaps in marriage and service determined the extent of benefits authorized.  Documents required are original or certified marriage certificate, original or certified copy of the divorce decree, sponsor's DD Form 214 or Statement of Service.  All others must process their request through their respective branch of service. 

100% Disabled Veterans:  Requires a letter from the Veterans Administration stating that he/she is 100% disabled service-connected.  The letter must state whether the DAV is scheduled for future examinations.  A DD Form 214, picture ID and social security card are also required.  

Retired Department of the Army Civilians:  SF 50 indicating the individual is retired and individuals will be required to have 2 forms of identification from the table above - one must have a photo. 

Incapacitated Child over age 21, Parent/Parent-In-Law Dependency (Army):  Dependency determination can be done for a child 21 years of age who is incapacitated: parents and parents-in-laws provided the sponsor is supporting these dependents with at least 50% of their living expenses.  Documents will be forwarded to DFAS who will make the final determination.  All others must process their requests through their respective branch of service.

    Incapacitated Child over 21 years of age:  DD Form 137-5 which can be obtained from the ID card office, birth certificate, medical statement verifying the child's medical incapacitation is permanent or temporary, letter from the social security office verifying whether the child is receiving social security benefits, and if so, whose social security number the benefits are being received under.

    Parent/Parent-In-Law Dependency:  DD Form 137-3, which can be obtained from the ID card office, the sponsor's or spouse's birth certificate reflecting  the parent's name (as applicable).  If the parent has medical problems, a medical statement is required.
